The active form (1,25-(OH)₂D₃) acts like a steroid hormone, binding to vitamin D receptors (VDR) on cells to regulate genes that control calcium/phosphate balance. 

Development of Therapeutic Vitamin D

Doctors began using vitamin D (both D₂ and D₃) in very large doses to treat bone-mineral disorders. For example, in renal osteodystrophy (a bone disease in kidney failure), doses of 150,000-500,000 IU were historically used, with careful monitoring of calcium and phosphate. 

Over time, clinicians developed injectable forms (intramuscular) of vitamin D3 (cholecalciferol) to deliver large “loading” doses. This is especially useful when oral supplementation is not practical or rapid correction is needed. 

Research has shown that single large doses (e.g., 300,000 IU) administered once can safely raise vitamin D levels in some populations.

Modern Understanding & Safety
